Indications
Xarelto (generic name: rivaroxaban) is a prescription oral film-coated tablet used to prevent blood clots and stroke. It belongs to a medicine class called anticoagulant. It works by inhibiting Factor Xa, which is involved in the formation of clots.

Uses and Dosage

- 5 mg twice daily, with or without food, in combination with aspirin (75 mg-100 mg) once daily. No dose adjustment needed based on CrCl.
For Deep Vein Thrombosis (DVT) Treatment
- For the first 21 days, 15 mg twice daily with food, at the same time each day in patients with a Creatinine clearance (CrCl ≥15 mL/min). Starting at day 22, change to 20 mg once daily with food, at the same time each day, for remaining treatment in patients with a CrCl ≥15 mL/min. Avoid use in patients with Creatinine clearance CrCl <15 mL/min.
For Reduction in the Risk of Recurrence
- 10 mg once daily with or without food after ≥6 months of standard anticoagulant treatment in patients with a CrCl ≥15 mL/min
For After Hip Replacement Surgery
- 10 mg once daily for 35 days, 6 to 10 hours after surgery once hemostasis has been established, with or without food, in patients with a CrCl ≥15 mL/min. Avoid use in patients with CrCl <15 mL/min.
For Acutely Ill Medical Patients
- 10 mg once daily, in hospital and after hospital discharge, for a total recommended duration of 31 to 39 days, with or without food, in patients with a CrCl ≥15 mL/min. Avoid use in patients with CrCl <15 mL/min.
