What is Cymbalta?

Cymbalta is a brand-name drug available only with a doctor’s prescription.   

It contains the active ingredient duloxetine, which belongs to a class of drugs known as selective serotonin and norepinephrine reuptake inhibitors (SNRIs). Cymbalta prevents serotonin (5-hydroxytryptamine) and norepinephrine/noradrenaline from being taken back into nerve cells in the brain and spinal cord. These neurotransmitters play a role in maintaining a high mood and decreasing the pain sensation. This action of Cymbalta raises the amount of these neurotransmitters in the spaces between these nerve cells, increasing the communication level between cells.   

Since Cymbalta can block the re-uptake of serotonin and norepinephrine neurotransmitters, it has been approved for use in the treatment of major depression, generalized anxiety disorder (GAD), diabetic peripheral neuropathic pain, fibromyalgia, and chronic musculoskeletal pain in adult patients. 

Cymbalta is also used to treat children who are at least 7 years old with a generalized anxiety disorder (GAD) and children at least 13 years old with fibromyalgia.

Cymbalta dosage

Cymbalta comes in a delayed-release capsule form and is available in three different strengths: 20 mg, 30 mg, and 60 mg. 

The following are the recommended Cymbalta dosages for each condition: 

For adults with Major Depressive Disorder: 

  • The starting dose is 40 mg (20 mg two times a day) to 60 mg, which is given either once per day or 30 mg twice daily.
  • Some patients start on Cymbalta 30 mg once a day for a week to allow them to adjust to the drug before increasing the dose to 60 mg once a day.
  • The maximum daily dose is 120 mg.
  • The maintenance dose is 60 mg once per day. Patients should be reassessed regularly to determine if continued maintenance treatment is needed and the proper dose for such treatment.

For adults with Generalized Anxiety Disorder (GAD) 

  • For most patients, the starting dose is 60 mg orally once daily. 
  • Some patients start on Cymbalta 30 mg once a day for a week to allow them to adjust to the drug before increasing the dose to 60 mg once a day.  
  • The maintenance dose is 60 to 120 mg once per day. Patients should be reassessed regularly to determine if continued maintenance treatment is needed and the proper dose for such treatment.  
  • The maximum dose is 120 mg per day.

For adults with Diabetic Peripheral Neuropathic Pain: 

  • 60 mg once daily, but for patients with tolerability concerns, consider a lower initial dose. 
  • For patients with renal impairment, a lower initial dose and gradual increase in dose must be considered.

For adults with Fibromyalgia or Chronic or Musculoskeletal Pain: 

  • 60 mg once per day, but treatment should start at 30 mg daily for a week to allow them to adjust to the drug before increasing the dose to 60 mg once a day.

For pediatric patients aged 7 to 17 years old with GAD: 

  • The starting dose is 30 mg daily for at least 2 weeks. 
  • The maintenance dose is 30 to 60 mg once per day.
  • The maximum dose is 120 mg per day.

For pediatric patients aged 13 years and older: 

  • The starting dose is 30 mg once a day for at least 1 week. 
  • The maintenance dose is 30 to 60 mg once per day.

Direction 

Take Cymbalta exactly as directed by your physician. Read the medication guides and instruction sheets and follow all the instructions on the prescription label. 

Cymbalta is an oral medication that can be taken on an empty or full stomach. 

Be sure to swallow the capsule whole and do not chew, crush, open, or break it. Do not mix this medication’s contents with food or liquid.

Caution

Do not take Cymbalta if you are allergic to its active and inactive components. 

Cymbalta should not be taken within 5 days before or 14 days after you used MAO inhibitors such as selegiline, rasagiline, phenelzine, linezolid, isocarboxazid, tranylcypromine, or methylene blue injection. 

Cymbalta should not be used by patients with narrow-angle glaucoma as it can lead to a serious condition known as serotonin syndrome. Inform your doctor if you are also using the following: 

  • Opioid medicine 
  • Herbal products 
  • Medication for depression, mental illness, migraine headaches, Parkinson’s disease, serious infections, or prevention of nausea and vomiting 

Cymbalta has not been approved for use in children under 7 years old. 

To ensure that Cymbalta is safe for you, tell your doctor if you have: 

  • Kidney or liver disease 
  • Heart problems, increased blood pressure 
  • Slow digestion 
  • A seizure (convulsion) disorder
  • Bleeding problems
  • Sexual problems
  • Bipolar disorder/manic-depressive disorder (psychiatric disorders)
  • Narrow-angle glaucoma
  • Drug addiction or thoughts of wanting to kill yourself
  • If you drink too much alcohol (alcohol abuse) 

Stay alert to your mood changes or symptoms, and your family members and caregivers must watch out for sudden behavior changes. 

Cymbalta (duloxetine) may affect the blood sugar of people who have diabetes. Therefore, check your blood sugar regularly and share it with your doctor. 

A pregnant woman is at a high risk of bleeding after giving birth if Cymbalta is taken in the month prior to delivery. It may also cause serious medical complications in the baby immediately upon delivery. 

If you are breastfeeding, inform your doctor if your nursing baby has drowsiness, feeding problems, and slow weight increase.

Cymbalta side effects

Like most medications, Cymbalta can cause some unwanted side effects along with its needed effects. Some may be serious; some may just simply go away after some time or the body has adjusted to the drug.

  • Liver damage is shown by itchy skin, dark-colored urine, pain in the upper right part of the stomach, and yellowing of the skin and/or white-colored part of the eyes (jaundice) 
  • Orthostatic hypotension is shown by fainting or dizziness when changing from a lying position to a sitting or standing position. This could lead to falls. 
  • Serotonin syndrome (increased amount of serotonin) is shown by agitation, muscle twitches, nausea, vomiting, diarrhea, fever, rapid heart rate, tremors, and sweating. 
  • Low levels of sodium in the blood (Hyponatremia) are shown by headache, confusion, weakness, difficulty concentrating, and memory problems.
  • Pain or difficulty urinating, decreased urine flow, and increased frequency of urination
  • Stevens-Johnson syndrome is a severe skin reaction shown by a peeling rash, mouth sores, hives, and blisters.
  • Glaucoma is shown by eye pain, vision changes, and redness or swelling in or around the eye/s.
  • Elevated blood pressure (Hypertension)
  • Unusual bleeding or bruising
  • Withdrawal symptoms
  • A serious allergic reaction with symptoms such as rash, itching, swelling of the face, lips, tongue, or throat, and trouble breathing.
  • Heart attack (myocardial infarction)
  • Suicidal thoughts or behaviors
