What is Janumet XR?

Janumet XR is a brand-name medication containing two antidiabetic medications to improve blood sugar control in adults with type 2 diabetes mellitus: metformin hydrochloride extended-release, belonging to the biguanide class, and sitagliptin, a dipeptidyl peptidase-4 (DPP4) inhibitor.  

This medication should be used along with a proper diet and regular exercise.

This medication requires a doctor’s prescription upon obtaining it.

Janumet dosage

Janumet XR comes in as an extended-release tablet with three different strengths: 50/500 mg, 50/1000 mg, and 100/1000 mg.

The dose of this medication should be individualized and based on the patient’s present treatment, tolerability, and effectiveness, while not going above the maximum daily dose of 100 mg sitagliptin and 2000 mg metformin.

The recommended daily dose of Janumet XR in patients not currently on metformin treatment is 100 mg sitagliptin and 1000 mg metformin hydrochloride extended-release. If this dose of metformin provides inadequate glycemic control, the dose can be gradually titrated up to the maximum recommended daily dose.

The recommended daily dose of Janumet XR in patients already on Metformin treatment is 100 mg sitagliptin and the dose of metformin was previously prescribed to them.

Directions

Take Janumet XR exactly as prescribed by your physician. Read all the medication guides and instruction sheets and carefully follow all the instructions on the prescription label.

This medication should be taken with food to lower gastrointestinal side effects associated with its metformin content. Preferably, this medication should be taken once daily with a meal in the evening.

This drug should be swallowed whole. Crushing, splitting, or chewing the tablet is not recommended.

You may experience low blood sugar (hypoglycemia) and feel dizzy, hungry, shaky, or confused. This can be treated quickly by eating or drinking fast-acting sources of sugar such as hard candy, fruit juice, crackers, and non-diet soda.

Watch out for increased blood sugar (hyperglycemia), such as increased thirst and urination. Blood tests will be needed often.

Cautions

Do not take Janumet XR if you are allergic to it or to any of its active and inactive components or if you have severe kidney disease or diabetic ketoacidosis.  

Before taking this medication, you must inform your doctor about your medical history, especially heart failure, kidney problems, liver problems, pancreatitis, gallstones, high triglycerides, and alcoholism.

Suppose you need to undergo surgery or any X-ray or CT scan using a dye or contrast agent that needs to be injected into your veins. In that case, you may need to stop using Janumet XR temporarily. Ensure you inform your healthcare provider that you are using this medicine.

Strictly follow your doctor’s orders about using Janumet XR if you are pregnant or become pregnant. It is important to control diabetes during pregnancy, and if blood glucose level is high, complications may occur in both the mother and the baby.

Premenopausal women taking metformin may have an increased risk of unintended pregnancy because metformin may stimulate ovulation. Talk with your healthcare provider and ask about your risk.
