Indications

Topiramate is used alone or with other medications to prevent and control seizures (epilepsy). This medication is also used to prevent migraine headaches and decrease how often you get them.

Topiramate will not treat a migraine headache once it occurs. If you get a migraine headache, treat it as directed by your doctor (such as by taking pain medication, or lying down in a dark room).

Topiramate is known as an anticonvulsant or antiepileptic drug.

Use and Dosage

Read the Medication Guide and, if available, the Patient Information Leaflet provided by your pharmacist before you start taking topiramate and each time you get a refill. If you have any questions, ask your doctor or pharmacist.

Take this medication by mouth with or without food as directed by your doctor, usually twice daily. You may swallow this medication whole, or you may open the capsule and sprinkle the contents on a teaspoonful of soft food (such as applesauce, or yogurt). Swallow the drug/food mixture right away. Do not chew the mixture. Do not prepare the mixture ahead of time for later use. Drink liquid after swallowing the mixture to ensure all the drug is swallowed.

To prevent kidney stones from forming, drink plenty of liquids while taking this medication unless your doctor instructs you otherwise.

Dosage is based on your medical condition and response to treatment. For children, the dosage is also based on weight. Your doctor will gradually increase your dose to reduce your risk of side effects. For some conditions, you may start treatment with topiramate once daily at bedtime and slowly increase your dose to twice a day. It may take several weeks or months to reach the best dose for you and to get the full benefit from this medication.

Take this medication regularly to get the most benefit from it. To help you remember, take it at the same time each day. Do not stop taking this medication without consulting your doctor. Some conditions may become worse when this drug is suddenly stopped. Your dose may need to be gradually decreased.

Side Effects

Get emergency medical help if you have signs of an allergic reaction (hives, difficulty breathing, swelling in your face or throat) or a severe skin reaction (fever, sore throat, burning eyes, skin pain, red or purple skin rash with blistering and peeling).

Tell your doctor right away if you have new or sudden changes in mood or behavior, including new or worse depression or anxiety, panic attacks, trouble sleeping, or if you feel impulsive, irritable, agitated, hostile, aggressive, restless, more active or talkative, or have thoughts about suicide or hurting yourself.

Serious side effects include:

  • A skin rash, no matter how mild
  • Dizziness or loss of coordination
  • Vision problems, blurred vision, eye pain or redness, or sudden vision loss (can be permanent if not treated quickly)
  • Confusion, problems with thinking or memory, trouble concentrating, or problems with speech
  • Dehydration symptoms–decreased sweating, high fever, or hot and dry skin
  • Signs of a kidney stone–severe pain in your side or lower back, or painful or difficult urination
  • Signs of too much acid in your blood–irregular heartbeats, feeling tired, loss of appetite, trouble thinking, or feeling short of breath
  • Signs of too much ammonia in your blood–vomiting, unexplained weakness, or feeling like you might pass out

Common side effects of Topamax Sprinkle may include:

  • Dizziness, drowsiness, tiredness, or slow reactions
  • Problems with speech or memory
  • Abnormal vision
  • Numbness or tingling in your arms and legs or decreased sensation (especially in the skin)
  • Changes in your sense of taste or feeling nervous
  • Nausea, diarrhea, stomach pain, or loss of appetite
  • Fever or weight loss
  • Cold symptoms such as stuffy nose, sneezing, and sore throat

Cautions

  • Before taking topiramate, tell your doctor or pharmacist if you are allergic to it; or if you have any other allergies. This product may contain inactive ingredients, which can cause allergic reactions or other problems. Talk to your pharmacist for more details.
  • Before using this medication, tell your doctor or pharmacist your medical history, especially of: a certain eye problem (narrow-angle glaucoma), kidney problems (such as kidney stones), liver problems, mental/mood problems (such as depression, thoughts of suicide), lung/breathing problems, a certain metabolic imbalance (metabolic acidosis), long-term diarrhea, a diet high in fat and low in carbohydrates (ketogenic diet), brittle bones (osteoporosis).
  • This drug may make you dizzy, drowsy, or impair your judgment. Alcohol or marijuana (cannabis) can worsen these effects. Do not drive, use machinery, or do anything that needs alertness until you can do it safely. Avoid alcoholic beverages. Talk to your doctor if you are using marijuana (cannabis).
  • This medication may make you sweat less, making you more likely to get heat stroke. Avoid doing things that may cause you to overheat, such as hard work exercise in hot weather, or using hot tubs. When the weather is hot, drink a lot of fluids and dress lightly. If you overheat, quickly look for a place to cool down and rest. Get medical help right away if you have a fever that does not go away, mental/mood changes, headache, or dizziness.
  • Before having surgery, tell your doctor or dentist about all the products you use (including prescription drugs, nonprescription drugs, and herbal products).
  • Children may be more sensitive to the side effects of this drug, especially weakened bones, slowed growth rate, and decreased sweating. Consult your doctor or pharmacist for more details.
  • Older adults may be more sensitive to the side effects of this drug, especially dizziness. Dizziness can increase the risk of falling.
  • During pregnancy, this medication should be used only when needed. It may harm an unborn baby. Discuss the use of reliable forms of birth control with your doctor (see also Drug Interactions section). If you are planning pregnancy, become pregnant, or think you may be pregnant, immediately talk to your doctor about the benefits and risks of using this medication during pregnancy. If you are taking this drug to treat seizures, note that untreated seizures are a serious condition that can harm both a pregnant woman and her unborn baby, so do not stop taking this medication unless directed by your doctor.
  • This medication passes into breast milk and may have undesirable effects on a nursing infant. Consult your doctor before breast-feeding.

What happens if I miss a dose?

Take the medicine as soon as you can. Do not take two doses at one time. Skip a missed Topamax dose if your next dose is due in less than 6 hours. Call your doctor if you have missed more than one dose.

What happens if I overdose?

Seek emergency medical attention. An overdose of Topamax Sprinkle can be fatal.

What should I avoid while taking topiramate?

Do not drink alcohol. Dangerous side effects or increased seizures may occur.

Avoid becoming overheated or dehydrated in hot weather. Topamax Sprinkle can increase body temperature and decrease sweating, leading to life-threatening dehydration (especially in children).

Avoid the use of a ketogenic or “ketosis” diet (high in fat, low in carbohydrates) while you are taking Topamax Sprinkle.

Topamax Sprinkle may cause blurred vision and may impair your reactions. Avoid driving or hazardous activity until you know how Topamax Sprinkle will affect you.

Also avoid activities that could be dangerous if you have an unexpected seizure, such as swimming or climbing in high places.
