Indications

Reclast is an osteoporosis medication given to those with a high risk of bone fracture to treat or prevent osteoporosis caused by menopause or steroids. This medication is also used to treat Paget’s disease.

Zoledronic acid is a bisphosphonate drug that promotes osteoclast cell death by inhibiting one of its essential intracellular enzymes. This action supports bone formation and strength by reducing osteoblastic resorption of bones.

This osteoporosis medication is an effective preventative measure for those at high risk of bone fracture and of developing osteoporosis. A study found that having once-yearly injections of Reclast in postmenopausal women with osteoporosis reduced their risks of bone fracture in their hips, spine, and other areas.

Uses and Dosage

The dose of Reclast administered will vary depending on the individual being treated. Follow all your doctor’s instructions for the best results.

  • Ensure that you are hydrated before injection
  • Do not inject Reclast if the solution has any visual particulates or if it is discoloured
  • Taking acetaminophen after the injection can reduce pain around the injection site
  • Postmenopausal women taking Reclast to treat osteoporosis usually take 5mg once yearly
  • To prevent osteoporosis in postmenopausal women, 5mg is usually given once every 2 years
  • For osteoporosis in men, usually 5mg is given once a year
  • To treat Paget’s disease, 5mg is usually given over a constant infusion rate
  • Injections should be administered over at least 15 minutes
  • You may need frequent tests to determine treatment length and kidney function
  • Reclast is often prescribed for 3 to 5 years
  • This medication is often accompanied by calcium and vitamin D supplements

Side Effects

Reclast may cause some side effects to develop while supporting your bone growth and density. Speak with your doctor for more information regarding any symptoms you may encounter when taking zoledronic acid.

Common Reclast side effects:

  • Trouble breathing
  • Nausea, vomiting, diarrhea, constipation
  • Bone, muscle, or joint pain
  • Fever or flu symptoms
  • Tiredness
  • Eye pain or swelling
  • Pain in your arms or legs
  • Headache
  • anemia

Serious Side Effects

Some more serious adverse reactions may also occur when taking zoledronic acid. Stop receiving injections and call your doctor immediately if you develop any of the following symptoms:

  • New or unusual hip or thigh pain
  • Jaw pain or numbness, red or swollen gums, loose teeth, slow healing after dental work
  • Severe joint, bone, or muscle pain
  • Kidney problems (little or no urination, swelling in feet or ankles, tiredness)
  • Low red blood cell count (pale skin, unusual tiredness, light-headedness, shortness of breath, cold hands and feet)
  • Low calcium levels (muscle spasms or contractions, numbness or tingling around your mouth of in your fingers and toes)

Serious side effects related to your kidney function may be more likely to occur in older adults taking Reclast.

Allergic Reactions

Do not use Reclast if you are allergic to zoledronic acid. Stop receiving injections and contact your doctor right away if you develop any signs of an allergic reaction.

Reclast allergy symptoms may include:

  • Hives

  • Swelling of face, lips, tongue, or throat

Cautions

Reclast can affect your kidneys and cause problems, especially if you are dehydrated, take diuretic medicine, or if you have kidney disease. Contact your doctor if you notice signs of kidney problems such as urinating less than usual, swollen feet or ankles, or if you are tired or short of breath.

Your doctor may recommend having a dental exam before starting treatment to prevent tooth or gum issues that may occur when taking zoledronic acid.

Avoid smoking or try to quit during your Reclast treatment. Smoking can affect your bone mineral density and increase your risk of fractures.

Avoid drinking large amounts of alcohol while taking Reclast because it can cause bone loss.

Some patients using similar medicines to Reclast reported bone loss in their jaws. Signs of osteonecrosis of the jaw include jaw pain, swelling, numbness, loose teeth, gum infections, and slow healing after dental surgery. This condition may be more likely to occur if you have cancer or have undergone chemotherapy, radiation, or steroidal treatment.

Do not receive injections of Reclast if you have either of the following:

  • Low calcium levels in your blood
  • Severe kidney disease

Do not administer Reclast if you also use any other bisphosphonate drugs such as alendronate, etidronate, or ibandronate.

To ensure Reclast is a safe treatment option for you, tell your doctor if you have ever had any of the following:

  • Kidney disease
  • Hypocalcaemia
  • Thyroid or parathyroid surgery
  • Surgery to remove part of your intestine
  • Asthma caused by aspirin
  • Any condition that reduces your body’s ability to absorb nutrients from food
  • Dental issues
  • If you are dehydrated
  • If you take diuretic medicine

Reclast can cause harm to your kidneys, especially if used at the same time as some medications for infections, cancer, osteoporosis, organ transplant rejection, bowel disorders, high blood pressure, pain, or arthritis.

Some other drugs can interact with zoledronic acid and cause adverse reactions. To ensure your safety when receiving Reclast injections, tell your doctor about all the medications you take, including over the counter medicine, vitamins, and herbal products.

Do not receive Reclast injections if you are pregnant. Zoledronic acid can cause harm to an unborn baby and lead to birth defects or complications. Ensure that you use effective birth control during treatment and for several weeks after your last dose.

Reclast may affect female fertility. It is still important to use effective birth control to prevent pregnancy during treatment.

Do not breastfeed while receiving Reclast injections.

FAQ

What is the difference between Prolia and Reclast?

Both medications are injections used to treat or prevent osteoporosis in women who have completed menopause. However, Prolia is a monoclonal antibody that supports bone health and inhibits bone resorption differently than Reclast which is a bisphosphonate. This medication helps kill osteoclast cells so bone mass and density can increase.

How should I prepare for my Reclast infusion?

Before receiving your Reclast injection, you should drink at least two glasses of water or another liquid within a few hours of the shot. This helps ensure that you are not dehydrated which offers some protection for your kidneys.
