Benicar HCT is a combination medicine is used to treat high blood pressure (hypertension). Hydrochlorothiazide is a thiazide diuretic (water pill) that helps prevent your body from absorbing too much salt, which can cause fluid retention. Olmesartan is an angiotensin receptor blocker (ARB). It can prevent blood vessels from becoming too narrow and will lower blood pressure and keep the blood flowing correctly.

Precautions
Periodic determinations of serum electrolytes to detect possible electrolyte imbalance should be performed at appropriate intervals. All patients receiving thiazide therapy should be observed for clinical signs of fluid or electrolyte imbalance: hyponatremia, hypochloremic alkalosis and hypokalemia.
Hypokalemia may develop, especially with brisk diuresis, when severe cirrhosis is present, or after prolonged therapy.
In diabetic patients dosage adjustments of insulin or oral hypoglycemic agents may be required. Hyperglycemia may occur with thiazide diuretics. Thus latent diabetes mellitus may become manifest during thiazide therapy.
If progressive renal impairment becomes evident consider withholding or discontinuing diuretic therapy.

Pharmacist Tips
- Twice-daily dosing offers no advantage over the same total dose given once daily.
- For patients requiring further reduction in blood pressure after 2 weeks of therapy, the dose may be increased to 40 mg.
- The side effects Benicar are generally rare and independent of dose; those of hydrochlorothiazide are most typically dose-dependent (primarily hypokalemia).

Can I take this with food?
Can be taken with or without food.
Can I take this if I am pregnant?
When used in pregnancy during the second and third trimesters, drugs that act directly on the renin-angiotensin system can cause injury and even death to the developing fetus. When pregnancy is detected, Benicar HCT should be discontinued as soon as possible.
Can I take this while breastfeeding?
It is not known whether olmesartan is excreted in human milk, but olmesartan is secreted at low concentration in the milk of lactating rats. Because of the potential for adverse effects on the nursing infant, a decision should be made whether to discontinue nursing or discontinue the drug, taking into account the importance of the drug to the mother.

Safety and effectiveness in pediatric patients have not been established.
Can this be titrated?
The usual recommended starting dose of Benicar (olmesartan medoxomil) is 20 mg once daily when used as monotherapy in patients who are not volume-contracted. For patients requiring further reduction in blood pressure after 2 weeks of therapy, the dose may be increased to 40 mg. Doses above 40 mg do not appear to have greater effect.
