Farxiga Drug Class 

This brand-name prescription medication belongs to a class of drugs called antidiabetics and sodium-glucose co-transporter 2 (SGLT2) inhibitors. This medicine works on the proximal renal tubules where sodium-glucose co-transporter 2 (SGLT2) is expressed. SGLT2 is responsible for the reabsorption of glucose back into the body.  

The inhibition of SGLT2 by Farxiga decreases the reabsorption of glucose from the proximal tubules, which results in an increased amount of glucose removed from the body through the urine. This helps in lowering glucose levels in patients with type 2 diabetes mellitus.  

Farxiga also reduces sodium reabsorption and increases the amount of sodium delivered to the distal tubule which influences some physiological functions such as reducing the pressure under which the heart fills (preload) and reducing the resistance the heart has to pump against (afterload). Also, inhibiting the sympathetic nervous system that is associated with heart failure, decreases intraglomerular pressure, and activates the tubuloglomerular feedback response for correcting glomerular hyperfiltration. 

Farxiga Dosage 

Farxiga comes as a film-coated tablet taken orally and is available in two strengths: 5mg and 10 mg. 

Before you start to use this medication, assess renal function, and then as clinically indicated, as well as, assess the volume status, and if needed, correct volume depletion. 

The recommended initial dose to improve glycemic control in patients with type 2 diabetes mellitus with an eGFR of >45 is 5mg orally once daily. The dose can be adjusted to 10 mg orally once a day for additional glycemic control if 5mg per day is tolerated. The recommended maximum dose is 10 mg orally once daily. 

For those with an eGFR of 25 to less than 45, the recommended dose is 10 mg orally once daily. 

For all other indications, the recommended initial dose is 10 mg orally once a day. 

What Are Faxiga Side Effects? 

Like most medications, Farxiga can cause some mild or serious side effects along with its needed effects.  

Common mild side effects 

  • Yeast infection in the genital area 
  • Respiratory infections (such as the flu and the common cold) 
  • Urinary tract infection (UTI)
  • Changes in urination such as increased urination, in larger amounts, or in the evening
  • Stuffy or runny nose and sore throat
  • Back pain
  • Nausea
  • Constipation
  • Elevated cholesterol levels 

These common mild side effects may go away after some time. If they persist and become bothersome or worse, talk to your doctor and ask for medical advice about managing them. 

Reported serious side effects 

  • Rash, itching, swelling of the face, lips, tongue, or throat
  • Abdominal pain, nausea and vomiting, tiredness, and trouble breathing
  • Dizziness, lightheadedness, feeling faint, confusion, feeling weak, especially when standing up (orthostatic hypotension), fatigue and thirst
  • Burning sensation or pain when you urinate, urinary frequency and urgency, pelvic (lower part of the stomach) or back pain, blood in the urine with or without fever, and nausea and vomiting
  • Shaking, sweating, dizziness, headache, confusion, rapid heartbeat, hunger, weakness, drowsiness, and irritability
  • Fever, feeling very weak, tired, or uncomfortable (body malaise), pain, tenderness, swelling, and redness of the skin in the genital or rectal area or areas around them (signs and symptoms of Fournier’s gangrene or necrotizing fasciitis of the perineum
  • Vaginal itching, vaginal odor, and white or yellowish vaginal discharge
  • Rash of the penis, redness, itching and/or swelling of the penis, foul-smelling penile discharge, and pain in the skin around the penis
