Cardizem SR is a medication that contains the active ingredient diltiazem.

Cardizem SR is primarily used to treat high blood pressure (hypertension), angina pectoris (chest pain), and certain types of heart rhythm disorders.

Cardizem SR is an extended-release capsule that delivers the medication slowly over time, providing 24-hour blood pressure control with once-daily dosing.

Use and Dosage

Take this medication by mouth with or without food as directed by your doctor, usually twice daily. Swallow the capsules whole. Do not crush or chew the capsules. Doing so can release all of the drugs at once and may increase your risk of side effects.

Your doctor may gradually increase your dose. Follow your doctor’s instructions carefully. The dosage is based on your medical condition, response to treatment, and other medications you may be taking. Be sure to tell your doctor and pharmacist about all the products you use (including prescription drugs, nonprescription drugs, and herbal products).

Use this medication regularly to get the most benefit from it. To help you remember, use it at the same times each day. Keep taking this medication even if you feel well. Most people with high blood pressure do not feel sick.

If this medication is used for angina, this medication must be taken regularly to be effective. It should not be used to treat angina when it occurs. Use other medications (such as nitroglycerin placed under the tongue) to relieve an angina attack as directed by your doctor. Consult your doctor or pharmacist for details.

It may take up to 2 weeks before you get the full benefit of this drug. Tell your doctor if your condition lasts or gets worse (for example, your routine blood pressure readings remain high or increase).

Side Effects

Get emergency medical help if you have signs of an allergic reaction to diltiazem (hives, difficulty breathing, swelling in your face or throat) or a severe skin reaction (fever, sore throat, burning eyes, skin pain, red or purple skin rash with blistering and peeling).

Serious side effects include:

  • Chest pain, fast, slow, or uneven heart rate
  • A light-headed feeling, like you might pass out
  • Heart problems – swelling, rapid weight gain, or feeling short of breath
  • Liver problems – loss of appetite, stomach pain (upper right side), tiredness, itching, dark urine, clay-colored stools, or jaundice (yellowing of the skin or eyes)

Common diltiazem side effects may include:

  • Swelling
  • Infections or flu symptoms
  • Trouble breathing
  • Headache, dizziness, or weakness
  • Slower heart rate
  • Pain, bruising, swelling, or irritation where the medicine was injected
  • Nausea or upset stomach
  • Rash

Older adults may be more sensitive to the side effects of this drug, especially dizziness, constipation, or swelling ankles/feet.

This drug passes into breast milk and may have undesirable effects on a nursing infant. Consult your doctor before breastfeeding.
