Indications

Parnate is a monoamine oxidase inhibitor (MAOI) medication used to treat episodes of major depression in those resistant to other treatment methods.

Tranylcypromine is an MAO inhibitor that increases the availability of chemicals in your brain such as serotonin, norepinephrine, dopamine, and trace amines. This action allows Parnate to treat and reduce episodes of major depression by balancing your brain chemicals.

This MAO inhibitor medication is a safe and effective form of treatment for major depression in those who are resistant to other forms of treatments. Parnate has a short half-life, but thanks to its non-reversible inhibition of monoamine oxidase, it offers pharmacodynamic effects for up to a week.

Uses and Dosage

The dose of Parnate that is prescribed will vary depending on the individual being treated. Follow all your doctor’s instructions and read packaging labels carefully to ensure the best treatment results.

  • Swallow tablets whole
  • Take doses at the same times each day
  • Your doctor may screen for bipolar disorder and blood pressure levels before treatment
  • Most adults take 30mg divided daily
  • Dosages can be increased by 10mg every 1 to 3 weeks
  • Adults should not take more than 30mg twice daily
  • Doses for those at risk of hypotension should be increased more gradually
  • If switching from a contraindicated antidepressant, wait at least one week or 4 to 5 half-lives of the medication before starting Parnate to avoid a drug interaction
  • Do not suddenly stop taking Parnate, speak with your doctor to gradually reduce doses
  • You may need to stop taking Parnate temporarily if you have surgery
  • Parnate may take several weeks to provide full benefits, continue using as prescribed
  • Store at room temperature away from children and pets

Cautions

Some young people who start using antidepressants like Parnate may have suicidal thoughts. Monitor your mood and behaviour and report any symptoms to your doctor.

Avoid eating foods that contain high amounts of tyramine during treatment with Parnate and up to two weeks after stopping because they can interact and lead to dangerously high blood pressure. Contact your doctor if you notice any symptoms such as a sudden and severe headache, nausea, vomiting, fever, cold sweat, dilated pupils, light sensitivity, fast or pounding heartbeats, neck stiffness, weakness, or problems with vision or speech.

Tyramine-rich foods to avoid include:

  • Air dried, aged, or fermented meats, including sausage or salami
  • Beer on tap (not from a bottle or can)
  • Pickled herring
  • Aged cheeses (blue, boursault, brie, camembert, cheddar, gruyere, mozzarella, parmesan, Romano, Roquefort, and Swiss)
  • Moldy or improperly stored meat, fish, poultry, or liver
  • Soy sauce, miso soup, bean curd, fava beans
  • Yeast extracts such as Marmite

Do not use Parnate if you have an adrenal gland tumour (pheochromocytoma).

Avoid driving or performing any hazardous activities until you know how Parnate will affect you because it could impair your reactions.

Tranylcypromine may interact with some drugs if taken within 14 days of each other. Do not take Parnate if you also use any of the following:

  • Other antidepressants
  • Buspirone, carbamazepine, reserpine, tapentadol, tetrabenazine, tryptophan
  • Diet pills, stimulants, ADHD medication, cough or allergy medicines
  • Another MAO inhibitor (isocarboxazid, linezolid, methylene blue injection, and others)
  • Drugs that affect your serotonin levels (stimulant medicine, opioid medications, herbal products, medicine for depression, mental illness, Parkinson’s disease, migraines, serious infections, or for preventing nausea and vomiting)

To ensure your safety when taking Parnate, tell your doctor if you have ever had any of the following:

  • Heart disease, high or low blood pressure, circulation problems, stroke
  • Bipolar disorder
  • Liver disease
  • Thyroid disorder
  • Headaches
  • Diabetes
  • Seizure

FAQ

What should be avoided when taking Parnate?

You should avoid foods that are high in tyramine during your Parnate treatment because they can interact and cause your blood pressure to rise to dangerous levels. These foods include smoked or pickled meats, dried fruits, beer that is on tap, aged cheeses, and more. You should also avoid driving or performing hazardous tasks until you know how this medication will affect you because tranylcypromine may impair your reactions.

How do I know if Parnate is working?

When you first start taking Parnate, it may take a few weeks before you feel the full effects of tranylcypromine. The medication is designed to help reduce the frequency and severity of episodes of major depression. Tell your doctor if you do not notice a change in your symptoms of depression after a month of treatment to reassess your treatment.
