WHAT IS COUMADIN? 

Coumadin (Warfarin) is a prescription drug that belongs to the class of drugs called anticoagulants. It is a blood thinner that helps treat and prevent the development of blood clots in your body. It helps to keep the blood flowing smoothly in your body to avoid strokes and heart attacks.  

It works by inhibiting Vitamin K on the liver to prevent clotting factors. Patients who use this drug should be continuously watched. 

COUMADIN PRECAUTIONS 

Before taking Coumadin, contact a healthcare professional if you are allergic to it, or have any other allergies. Tell them if you have a family or medical history of: 

  • Kidney Disease 
  • Liver Disease 
  • Alcohol Use 
  • Diabetes 
  • Stroke 
  • Recent Accident/Surgery 
  • Congestive Heart Failure 
  • Mental or Mood Disorders 
  • Frequent Falls/Injuries 
  • Low Blood Platelets 
  • Blood Vessel Disorders (Aneurysm) 
  • Blood Disorders (Anemia, Hemophilia) 
  • Bleeding Problems (Gastrointestinal Bleeding, Brain Hemorrhage) 

Before taking Coumadin, please contact your doctor first if you are pregnant or planning to become pregnant. Taking Coumadin while pregnant is not recommended as it may cause harmful or fatal effects on the unborn baby. Talk to your doctor about taking birth control one month after withdrawing from this medication.  

Also, please contact your doctor first if you are breastfeeding or planning to breastfeed. The effects of this medication on a nursing baby are still unknown. If you are taking Coumadin, watch for signs of bruising or bleeding in the nursing baby.

Taking an injection into the muscles while taking Coumadin is not recommended. The injection should be given in the arm instead. 

Avoid or use sharp objects with caution to avoid getting cut or bruised, as Coumadin can cause heavy bleeding.  

Geriatric (65 years or older) patients have a high risk of bleeding when taking this medication. 

Monitor the INR of the patient taking Coumadin regularly. 

Grapefruit, cranberry, pomegranate, and their products may cause interactions with Warfarin that may increase the risk of catching serious side effects. Avoid consuming the juice products of these fruits. 

Consuming green, leafy vegetables, alcoholic beverages, cranberry or cranberry products, and liver products should be limited or avoided when taking this medication. Call your doctor to discuss a healthy and detailed diet plan. 

COUMADIN SIDE EFFECTS 

The common side effects of Coumadin are: feeling cold, headache, bleeding, bruising, slow healing wounds, nausea, loss of appetite, and stomach/abdominal pain. Stop using this medication and seek medical attention immediately if you start experiencing serious side effects such as:

COUMADIN DOSING

For patients with Stroke and Thromboembolism, the recommended starting dose is 10 mg, taken orally, once a day, for 2 days. INR should be checked after the initial dosage. The dosage adjustment will be according to the results. The recommended maintenance dose is 2 mg to 10 mg once a day. 

For patients with Cardiac Valve Replacement, the recommended starting dose is 10 mg, taken orally, once a day, for 2 days. INR should be checked after the initial dosage. The dosage adjustment will be according to the results. The recommended maintenance dose is 2 mg to 10 mg once a day.  

For patients with post-myocardial Infarction, the recommended starting dose is 10 mg, taken orally, once a day, for 2 days. INR should be checked after the initial dosage. The dosage adjustment will be according to the results. The recommended maintenance dose is 2 mg to 10 mg once a day.
