What is Pradaxa?

Pradaxa is a brand-name prescription drug belonging to a class of drugs called thrombin inhibitors; Anticoagulants, Cardiovascular; Anticoagulants, Hematologic.

Pradaxa generic name and active ingredient is Dabigatran Etexilate. This medication works by blocking thrombin (an enzyme that’s responsible for blood clotting in the body), resulting in the prevention of clots.

Because of Pradaxa’s mechanism of action, it is used for:

-Reducing the risk of stroke due to blood clots in the brain and systemic embolism in adults who have non-valvular atrial fibrillation (abnormal heartbeat that is not caused by a heart valve problem).

-Treating adults with deep vein thrombosis (blood clots in the veins of the legs) and pulmonary embolism (blood clots in the lungs) who have been treated with a parenteral medication to treat blood clots (anticoagulant) for 5-10 days.

-Prevention of deep vein thrombosis (DVT) can lead to pulmonary embolism in patients who have undergone hip replacement surgery.

-Treating pediatric patients aged 8 to 17 years old with venous thromboembolic events (VTE) who have been treated with a parenteral medication to treat blood clots (anticoagulant) for 5 days. Also, to lower the risk of recurrence of VTE in patients included in the said age group.

Pradaxa dosing

Pradaxa comes in a capsule taken orally and is available in three different strengths: 75 mg, 110 mg, and 150 mg. The dose given depends on the health status of the patient’s kidneys, measured by a creatinine clearance test.

When used to reduce the risk of stroke and systemic embolism in adults who have non-valvular atrial fibrillation, the following are the recommended dosages.

-For adult patients who have more than 30 mL/min creatinine clearance (CrCl), Pradaxa 150 mg is taken by mouth (orally) two times a day.

-For adult patients who have severe renal impairment with a CrCl of 15-30 mL/min, Pradaxa 75 mg is taken by mouth twice a day.

-For adult patients with a CrCl of less than 15 mL/min or on dialysis, dosing recommendations cannot be provided.

Take Praxada exactly as directed by your healthcare provider. Read all the medication guides or instruction sheets and follow the instructions on the prescription label. This medication is taken on an empty or full stomach. You should swallow this medication whole with a full glass of water and it should not be chewed, crushed, broken, or opened. Do not take this drug more often or increase your dose than directed.

Cautions

Pradaxa may cause you to bleed since it is blood thinner, so don’t take this drug if you:

  • Presently experience abnormal bleeding
  • Have had a serious allergy to this drug and any of its ingredients
  • Have had a heart valve replacement or plan to do so

The following has a higher risk of bleeding with Pradaxa:

  • People aged 75 years and older
  • With kidney problems
  • Had recent or have recurrent stomach or intestinal bleeding or have a stomach ulcer
  • Those who take other medications that increase the risk of bleeding such as non-steroidal anti-inflammatory drugs (NSAIDs), blood thinners, and aspirin.
  • Have kidney problems or reduced kidney function
  • Currently taking dronedarone
  • Currently taking ketoconazole or itraconazole (medications for fungal infections)

If you will have surgery or any medical/dental procedures, inform your doctor that you are taking Pradaxa and all the medications you use, including non-prescription and prescription drugs, vitamins, nutritional supplements, and herbal products.

If you are pregnant or planning to become pregnant, or you are breastfeeding your child, inform your doctor. This medication may increase the risk of bleeding during labor and delivery, in the unborn baby, and the newborn baby.

Drinking alcohol daily while using this medication may increase your risk of stomach bleeding because this medication may cause bleeding in the stomach. To reduce the chance of getting injured or cut, be very careful when using sharp tools like razors, scissors, and nail cutters. Avoid activities like contact sports.

Pradaxa foods to avoid

There is no known food that interacts with Pradaxa. You may continue to eat the food you like to eat while using Pradaxa. However, if you have kidney problems, it is recommended to avoid grapefruit while using Pradaxa. Grapefruit may increase the level of Pradaxa in the body which may increase the risk of bleeding. Ask your healthcare provider if there is a safe number of grapefruit you can consume while taking Pradaxa.

Frequently Asked Questions

-Can I drink alcohol while on Pradaxa?

There are no known interactions between Pradaxa and alcohol. However, alcohol, especially large amounts of it in the body, can increase the risk of stomach bleeding.

What will happen if I stop taking Pradaxa?

If you stop taking Pradaxa without consulting your prescribing doctor, you will increase your risk of forming blood clots, just like how you formed blood clots before using this medication. This could result in a higher risk of having a stroke. It’s important to talk to your doctor first before you decide to stop using Pradaxa.

Will I need to stop using Pradaxa before dental treatment?

There is no need to discontinue using Pradaxa as long as the dental procedure has a low bleeding risk such as simple exodontia (removal of teeth) and oral surgery lasting for 45 minutes. However, if you are at a greater risk of bleeding or the procedure has medium to high bleeding risk such as multiple extractions. Your dentist may tell you to stop taking Pradaxa. So, it is important that you tell your dentist that you are taking Pradaxa before having any dental treatment.
