What is Janumet? 

Janumet (generic name: sitagliptin and metformin hydrochloride) is an oral prescription drug indicated to improve glycemic control or lower elevated blood sugar levels in adult patients with type 2 diabetes mellitus. This is given as an adjunct to proper diet and exercise. 

By lowering blood sugar, this medication helps prevent diabetes complications, such as heart attack, stroke, blindness, and kidney problems. 

This oral drug is a combined dipeptidyl peptidase-4 (DPP-4) and biguanide. 

This is available in Janumet 50/1000 mg, 50/850 mg, and Janumet 50 500 mg tablets. 

How does Janumet work? 

This drug exerts its action by slowing the inactivation of incretin hormones. These hormones increase the release of insulin and decrease the level of glucagon in a glucose-dependent manner. This medication also has the ability to decrease hepatic glucose production, lessen the absorption of intestinal glucose, and enhance insulin sensitivity through the increase of peripheral glucose uptake and utilization. 

How should I take Janumet? 

To take this oral medication, you must first read the patient information leaflet. Take this medication exactly as directed. This is usually prescribed twice daily, every 12 hours. It is best to take this prescription drug with a meal. Swallow the tablet and drink plenty of fluids right after. Do not increase or decrease your Janumet dosage or take it more frequently than advised. 

Janumet side effects 

The common side effects of this oral antihyperglycemic are nausea, vomiting, dizziness, headache, fatigue, weakness, metallic taste in the mouth, stomach upset, diarrhea, constipation, muscle pain, and joint pain. Further, the severe adverse effects are low blood sugar (hypoglycemia), high blood sugar (hyperglycemia), changes in the amount of urine, unusual skin blisters, loss of appetite, severe abdominal pain, persistent vomiting, shortness of breath, sudden weight gain, swelling of extremities, and unusual tiredness. Call your doctor if this happens. 

Contraindications /Precautions  

This prescription medication should not be used if you have a known allergy to sitagliptin, metformin, or any inactive ingredients it contains. Signs of serious allergic reactions are skin reaction, severe dizziness, swollen lymph nodes, trouble breathing, swelling of the face and neck, and itchiness. If this occurs, seek emergency medical attention immediately. 

This medication is contraindicated for people with a history of breathing problems, blood problems, liver disease, kidney disease, pancreatitis, and gallstones. Due to drug interaction, you should also talk about all the prescription drugs, nonprescription drugs, supplements, and herbal products you are using. 

Drinking alcohol and diarrhea can further increase your risk of developing a condition called lactic acidosis. Avoid alcohol and water pills. You should also stay hydrated when taking this drug unless contraindicated. Stop taking Janumet if you have prolonged diarrhea or persistent vomiting. 

Due to its metformin component, this drug can affect the menstrual cycle and promote ovulation which increases the chance of getting pregnant. If needed, discuss some reliable birth control methods with your healthcare provider.
