What is Rapamune (Sirolimus)?

Rapamune (Sirolimus) is a prescription medication that helps prevent rejection in patients 13 years of age and older who have recently received a kidney transplant. This medication weakens the immune system to prevent it from attacking the transplant. It belongs to a drug class called mammalian target of rapamycin (mTOR) inhibitors or selective immunosuppressants. Rapamune is also known for helping in the treatment of a rare lung disease called lymphangioleiomyomatosis (LAM), the abnormal and progressive growth of muscle cells in the lungs and lymphatic system.  

What is Rapamune used for?

Rapamune is used as an anti-rejection medicine post-transplantation for the kidney. It weakens the body’s immune system to prevent it from recognizing the transplant as a foreign invader and attacking it.  

Common Side Effects of Rapamune 

The common side effects of Rapamune include dizziness, trouble sleeping (insomnia), acne, cold symptoms, shakiness (tremor), chest pain, joint pain, muscle pain, nausea, and diarrhea. Stop taking Rapamune, report side effects, and seek medical attention if you start experiencing:

Before taking Rapamune, contact a healthcare professional if you are allergic to the ingredients of this medication, or have any other allergies. Inform your healthcare provider further if you or a member of your family has suffered or is suffering from the following medical conditions.

  • Heart Disease
  • Liver Disease

Rapamune puts you at an increased risk of catching infections. Avoid contact with people who have contagious infections such as flu, measles, or chickenpox.  

Tell your healthcare professional that you are taking Rapamune before getting immunizations or vaccinations. Avoid contact with people who recently had live vaccines, such as flu vaccine, measles, rotavirus, mumps, and chickenpox.  

Rapamune puts you at an increased risk of developing skin cancer. Limit your time outside, and avoid direct contact with sunlight. Do not use tanning booths or sunlamps, and always use sunscreen and protective clothing before going outside.  

Before undergoing any surgery or dental procedure, make sure the doctor or dentist is aware that you are taking Rapamune. 

Rapamune can impair male fertility. Consult your doctor about this side effect.  

Inform your healthcare provider if you are pregnant or planning to become pregnant. Rapamune must not be taken while pregnant, and you must not get pregnant while taking this medication. It can harm an unborn baby and cause complications.  

Consult your healthcare provider about the use of reliable forms of birth control such as pills, condoms, and more. Consult your healthcare provider right away if you are experiencing symptoms of pregnancy.  

Inform your healthcare provider if you breastfeed or plan to breastfeed. This medication passes into breast milk and poses harm to a nursing infant. Breastfeeding while taking Rapamune is not suggested.

Rapamune Dosage

Act in accordance with your healthcare provider’s directions concerning the prescribed dose for your medical condition. Rapamune comes in tablet form. The Rapamune tablet comes in two dosage strengths, which are 1 mg, and 2 mg. 

The recommended dose for adults at low to moderate immunologic risk depends on their body weight. If the patient is less than 40 kg, the loading dose must be 3 mg/m2 on day 1, and the maintenance dose must be 2 mg, taken orally once daily. If the patient is equal to or greater than 40 kg, the loading dose must be 6 mg, taken orally once daily, and the maintenance dose must be 2 mg, taken orally once daily. 

The recommended dose for adults at high immunologic risk depends if they are taking Cyclosporine. For patients taking Cyclosporine together with Rapamune, the loading dose is 15 mg, on day 1 after transplantation. The maintenance dose must begin on day 2, with an initial dose of 5 mg per day.  

The recommended dose for adults with pulmonary lymphangioleiomyomatosis is 2 mg per day.  

The recommended dose for pediatric patients for rejection prophylaxis depends on their body weight. If the patient is less than 40 kg, the loading dose must be 3mg/m2 on day 1, and the maintenance dose must be 1mg/m2 once daily. If the patient is equal to or greater than 40 kg, the loading dose must be 6 mg, taken orally once daily, and the maintenance dose must be 2 mg, taken orally once daily.  

Rapamune levels in the blood must be checked and measured every 5 days to prevent toxicity. For further information, speak with your healthcare physician. 

In case you missed a dose of this medication, take it as soon as you remember. However, if your next planned dose for your condition is approaching, skip the missed dose and take the following one. Don’t take a double dose of Rapamune to make up for the missed dosage. 

In case you are experiencing fainting or trouble breathing, which are possible symptoms of an overdose, call the local emergency hotline and seek medical attention immediately.
