Leucovorin is prescribed to reduce the toxic effects of methotrexate and other antifolate medications in cancer treatment, and for certain folate deficiency conditions. Per PharmacyChecker International Pharmacy Verification Program Standards, a valid prescription is required to purchase leucovorin. A September 2025 United States FDA announcement has spurred confusion, an uptick in its off-label use, and, ultimately, shortages of this critical chemotherapy rescue therapy.
Leucovorin has not been approved to treat autism spectrum disorder (ASD) by any regulatory authority anywhere in the world.
There is very limited evidence supporting leucovorin to treat ASD. The American Academy of Pediatrics does not recommend the routine use of folinic acid to treat ASD. Ultimately, in March 2026, the FDA approved leucovorin for treating cerebral folate transport deficiency but not for treating ASD.
Due to shortages in the United States, the FDA is allowing “unapproved” importation of Lederle Leucovorin (calcium folinate tablets) marketed by Pfizer in Canada. The Canadian product is manufactured by Pfizer's Contract Manufacturer, Farmasierra, in Madrid, Spain.

Laws affecting the importation of medication for personal use are different from country to country. For instance, personal drug importation is expressly legal in Australia.
